چکیده
Abstract Background ECMO is widely used in critically-ill patients that require respiratory and/or cardiac support, but may affect drug exposure by altering pharmacokinetics within or sequestering drugs the circuit. Cefiderocol, a siderophore antibiotic with activity against multi-drug resistant gram-negative bacteria, be to treat serious infections supported ECMO. Limited data are available guide dosing this population. Herein, we determined loss of cefiderocol through an ex vivo Methods A closed-loop circuit prepared Quadrox-i adult oxygenator and primed Isolyte S was during all experiments. Fresh whole blood collected from healthy volunteers pooled, injected replace priming fluid, circulated at 4.5 L/min 37° C. Cefiderocol administered into 0h obtain peak plasma concentrations ∼89.7 mg/L. pre-oxygenator sample taken confirm baseline concentration. Post-oxygenator samples were between 0-24h determine control jar same pooled matrix spiked maintained C for comparison. Each ECMO/control experiment performed triplicate. general linear model constructed compare rate controls. Results Baseline ranged 51.7-91.8 mg/L 67.7-100.5 mg/L, respectively. At 0h, difference pre- vs post-oxygenator concentration -4 ± 4% (range: 0-7%). After 24h, % reduction similar circuits controls (50% 13 50% 9, p=1.0). The degradation followed zero order process did not differ significantly (slope: -0.0186 0.0052 -0.0201 0.0039, p=0.786). Conclusion sequestered studied oxygenator, suggesting insignificant removal can expected. Pharmacokinetic studies critically ill warranted final recommendations Disclosures David P. Extracorporeal membrane oxygenation (ECMO) reconsidered.
The role of extracorporeal membrane oxygenation (ECMO) in the treatment of the acute respiratory distress syndrome (ARDS) is controversial, notwithstanding the recent publication of the results of the CESAR (Conventional Ventilation or ECMO for Severe Adult Respiratory Failure) trial. Using Bayesian meta-analytic methods from three randomised controlled trials (RCTs) of ECMO in ARDS, we estimat...
متن کاملQuantification of perflutren microsphere contrast destruction during transit through an ex vivo extracorporeal membrane oxygenation circuit.
BACKGROUND Echocardiography is a key investigation in the management of patients on extracorporeal membrane oxygenation (ECMO). However, echocardiographic images are often non-diagnostic in this patient population. Contrast-enhanced echocardiography may overcome many of these limitations but contrast microspheres are hydrodynamically labile structures prone to destruction from shear forces and ...
متن کاملNeonatal respiratory extracorporeal membrane oxygenation (ECMO) referrals.
Extracorporeal membrane oxygenation (ECMO) is a complex technique for providing life support in neonatal respiratory failure. T UK Collaborative ECMO trial demonstrated cost-effectiveness and substantial improvements in neurological morbidity and mortality. Currently, infants requiring ECMO in Ireland are referred to one of various centres in the UK and Scandinavia. We aimed to review the numbe...
متن کاملImaging adults on extracorporeal membrane oxygenation (ECMO)
Extracorporeal membrane oxygenation (ECMO) is increasingly being used in adults following failure to wean from cardiopulmonary bypass, after cardiac surgery or in cases of severe respiratory failure. Knowledge of the different types of ECMO circuits, expected locations of cannulas and imaging appearance of complications is essential for accurate imaging interpretation and diagnosis. Commonly en...
متن کاملApplication of extracorporeal membrane oxygenation (ECMO) in tracheal tumor resection
Objective: Normal gas exchange and sufficient oxygenation are key preconditions for tracheal tumor resection. Due to the complex of diseases, conventional airway management during anesthesia, in some special cases, may meet some difficulties or risks. Then, some other options should be considered. We presented three cases where extracorporeal membrane oxygenation (ECMO) was used for cardiopulmo...
